Reviewing your care needs

What is a Review?


A Review is:

  • a re-assessment of your present care needs, and a check to make sure that the services you are receiving are still right to meet those needs
  • an opportunity for you to say what you think your present care needs are, and if the services you receive meet them
  • a chance for your carer(s), your Social Care Worker, and the people who provide your services to give their views.
 

Who will take part in my Review?


Your review will always involve you and your Social Care Worker. They usually involve your carer, let us know if you want your carer involved. A Review may also involve the people who provide the services you use.
 

How will my Review take place?


It may be:
 

  • a telephone conversation between you and your social care worker
  • a meeting with you, your Social Care Worker and perhaps some of the people providing your services
  • a written questionnaire your Social Care Worker asks you to complete
 

Will I be given anything in writing?


Your Social Care Worker will give you a copy of the Review Report.
What will my Review Report include?
 

  • A summary of what has been talked about during the review process
  • What your present care is
  • What services you will now receive
  • The month and year of when your next review is due


If there is a change in the services you receive, we will give you a new written Care Plan. This will say what services are being provided, any charges involved, and what date they will start from.
 

How will I know the date of my next Review?


Your Review Report will tell you the month and year of your next review.
 

How often will my Reviews take place?


This depends on the kind of care needs and services you have, for example everyone receiving services arranged by us should have a review at least once a year. However, if we are involved in paying for you to live in a residential or nursing home, there will be:
 

  • a review meeting at the home after the first 4 weeks
  • annual reviews thereafter.

 

What happens if my needs change before the date of my next Review?


Please tell your social care worker at once. Your care needs can be reviewed at any time, just ask and an interim review will be arranged for you.
 

Keeping your records safe


Records of your assessments and reviews will be stored on our computer, we will protect the confidentiality of the information we have about you. If you want to see your records, you should write to the Director of Adult Social Services
